Hayden Panettiere’s Death Linked to Fentanyl and Multiple Drugs

Hayden Panettiere died from the toxic effects of fentanyl and several other drugs, according to the Greenville County Coroner’s Office in South Carolina. The manner of death was ruled accidental.

Panettiere, who was 36, died on August 16 after she was found unresponsive at a residence in Greenville. Emergency responders had been called to the property following a report of cardiac arrest. Attempts to revive her were unsuccessful.

The coroner’s finding marks the first official determination of what caused the actress’s death after weeks of investigation and toxicology testing.

Coroner identifies multiple substances

The official finding lists fentanyl, 4-ANPP, alprazolam, methocarbamol and quetiapine as contributing substances.

Alprazolam is the generic name for Xanax. Methocarbamol is a muscle relaxant, while quetiapine is a prescription medication used for several medical conditions.

The coroner’s report did not classify Panettiere’s death as intentional. Instead, the manner was recorded as accidental.

An autopsy previously found no signs of trauma that would have contributed to her death.

What happened on August 16

Panettiere was staying temporarily at an apartment complex in Greenville, South Carolina, when emergency personnel were dispatched on August 16.

The Greenville County Coroner’s Office said a 911 call reporting a cardiac arrest was received at about 1:51 p.m. First responders found Panettiere in cardiac arrest and began advanced life-support measures. She was pronounced dead at approximately 2:32 p.m.

When the autopsy was completed the following day, officials said there were no signs of trauma that contributed to the death. At that stage, however, the cause and manner remained pending further investigation and additional testing.

Police also said their preliminary investigation did not indicate foul play or suspicious circumstances.

Investigation continued after her death

The official conclusion follows a separate investigation into the drugs that may have been involved.

Earlier in September, reports from law-enforcement sources indicated that investigators were examining whether Panettiere had obtained counterfeit prescription medication and whether a pill containing fentanyl could have played a role in her death.

At that time, the Greenville County Coroner’s Office had not yet released its toxicology findings. Local reporting on September 14 and 15 specifically noted that the official cause and manner of death remained pending.

That changed with the coroner’s latest determination.

The investigation also involved federal drug authorities, with the Drug Enforcement Administration examining the possible source of controlled substances connected to the case. Those investigative efforts are separate from the coroner’s determination of the medical cause of death.

The official finding establishes the substances identified in Panettiere’s system and classifies the death as accidental. It does not, by itself, establish that a particular individual supplied the drugs or that a particular counterfeit pill caused her death.

A career that began in childhood

Panettiere became a familiar face to television audiences after starting her career as a child performer.

She gained international recognition as Claire Bennet on NBC’s science-fiction drama Heroes, which premiered in 2006. She later starred as country singer Juliette Barnes on Nashville, a role that earned her two Golden Globe nominations.

Her film credits included Remember the Titans, Ice Princess, and entries in the Scream franchise.

Panettiere was also open in later years about personal struggles involving addiction and postpartum depression. Those experiences became part of the public discussion surrounding her career and personal life.

In her memoir, This Is Me: A Reckoning, published earlier in 2026, she wrote about her childhood in the entertainment industry, motherhood, and the difficulties she faced with addiction and recovery.

Earlier reports were not yet definitive

The timeline of the investigation is important because several reports circulated before the official toxicology determination.

In mid-September, reports citing people familiar with the investigation said authorities were examining a possible fentanyl-laced oxycodone pill. The Greenville County Coroner’s Office subsequently emphasized that it had not released an official cause or manner of death and was still awaiting toxicology results.

Those earlier reports should therefore be distinguished from the finding released on September 22.

The coroner has now officially attributed Panettiere’s death to the toxic effects of fentanyl and the other substances listed in the report and classified the death as accidental.

What remains under investigation

The medical cause of death has been established, but questions surrounding the origin of the drugs remain part of the broader investigation.

Authorities had been looking into where controlled substances came from and whether anyone could face consequences in connection with their distribution. The official coroner’s finding does not announce criminal charges against a supplier.

Panettiere’s death was initially investigated by local authorities and the Greenville County Coroner’s Office, with the DEA also involved in the examination of possible drug sources.

For now, the confirmed medical finding is that the 36-year-old actress died from the toxic effects of multiple substances, including fentanyl, and that the death was accidental.

Panettiere is survived by her daughter, Kaya, whom she had with former fiancé Wladimir Klitschko.

Her death brings the investigation into its cause to an official conclusion, while any separate inquiry into the source or distribution of the drugs may continue.
